Medicare Marketing Disclosures
We do not offer every plan available in your area. Currently we represent the organizations listed at the bottom of every page, which offer the number of products shown there in your area. Please contact Medicare.gov or 1-800-MEDICARE to get information on all of your options. No-cost, unbiased counseling is also available from your State Health Insurance Assistance Program (SHIP).
Vitality Health Advisors is not connected with or endorsed by the United States government or the federal Medicare program. Medicare Advantage and Part D plans are offered by private insurance companies with a Medicare contract; enrollment depends on contract renewal. Benefits, premiums, deductibles, copays, provider networks and formularies vary by plan and county and may change on January 1 of each year. Plan information on this site is a summary; the plan's Evidence of Coverage governs.

Call Recording
Medicare rules (42 CFR 422.2274(g)) require third-party marketing organizations to record all marketing and sales calls with beneficiaries, including the audio of web-based calls, in their entirety, and to retain them for at least six years. All calls to and from our agents are recorded. Information you submit on this site will be provided to a licensed insurance agent for future contact.
How We Are Paid
We receive commissions from insurance companies when you enroll in a plan through us. Agent compensation for Medicare Advantage and Part D plans is regulated by CMS. For Medicare Supplement and other insurance products, commissions vary by carrier and product. You never pay us a fee, and your premium is the same whether you enroll through an agent, the carrier or Medicare.gov.

Vitality Health Advisors LLC operates as the Medicare division of the Novavida family of companies, which includes Novavida Group (novavidagroup.com) and Instant Online Insurance Group. Vitality On Demand (vitalityondemand.com) is an affiliated consulting company and is not an insurance agency. If you give us permission on a form, we may share your contact information with the affiliated agencies named in that consent so that one of them can contact you; we do not sell your personal information otherwise.
